Contracted Healthcare Providers

At contracted private healthcare providers, medical services are delivered by staff physicians who are recognized as contracted by the Social Security Institution (SSI) and fall within the scope of General Health Insurance (GHI). For healthcare expenses covered by SSI at contracted rates, payment authorization is issued by the insurer in line with the benefit schedule. The insured is responsible only for the SSI co-payment and any amounts that fall outside the scope of coverage.

Non-Contracted Healthcare Providers

In cases where the insured applies to healthcare providers that are not contracted with the Social Security Institution (SSI) but are accepted by the insurer, the following documents must be submitted to the insurance company in order to enable an assessment of the healthcare expenses incurred by the insured:

  • Private Health Insurance Patient Information Form (the relevant sections must be completed and signed by the insured, the physician, and the healthcare provider where treatment was received),
  • A document approved by SSI showing the amount authorized by SSI for the indemnity payment made within the scope of GSS, along with a detailed breakdown (a detailed breakdown is not mandatory for medical material invoices),
  • Original invoices for all healthcare expenses and detailed invoice breakdowns (on a per-procedure basis),
  • Surgical report and/or discharge summary (epicrisis report) for inpatient treatments,
  • Test results related to the diagnosis of the condition,
  • Original paranasal sinus tomography of the insured prior to sinus surgery,
  • Official reports and documents issued by judicial authorities for any forensic incidents (including traffic accidents), such as alcohol reports, accident scene investigation reports, forensic medical reports, traffic accident determination reports, etc.
